📋 岗位职责 / 任职要求
岗位职责:
和睦家药剂科是一支专业化、国际化的药学服务团队,药师能够走出药剂科,成为多学科医疗团队的重要一员,保障患者从入院到出院的用药安全。科室特色服务包括:
药物重整:在患者入院及出院时,临床药师进行药物重整,收集患者处方药、非处方药、营养补充剂及过敏史,评估住院医嘱,保障药物治疗的准确性和连贯性。
疼痛管理:药师通过收集患者疼痛药物史,进行阿片类药物耐受性筛查,为医生制定疼痛管理方案提供参考,并指导护士评估患者疼痛。
处方前置审核:对每张处方进行前置审核,为患者用药把关,拦截潜在的重复用药和药物相互作用。
临床药学服务:临床药师参与多学科团队查房和药师查房,及时解决与用药相关的问题,为慢性病多重用药患者提供持续监测。
用药咨询:为患者提供处方药、非处方药及疫苗的用药指导,详细解释药物使用方法、注意事项及可能的不良反应。
工作范围
(一)门诊药房工作
审核和解读门诊部、急诊科的电子处方,评估并解决实际或潜在的处方相关问题。
按照门诊药房工作流程执行日常配药工作,包括药品调配、核对、发药、退货及欠单处理。
为患者提供处方药、疫苗及非处方药的用药咨询和指导服务。
参与药房日常运营工作,包括药品清点、效期检查、库存管理及各项记录维护。
(二)住院药房工作
审核住院患者用药医嘱,拦截可能的重复用药、药物相互作用及不合理用药。
负责住院患者药品的调配、核对与发放,确保药品准确及时送达病区。
在患者入院时收集药物治疗史,包括处方药、非处方药、营养补充剂及过敏史,完成药物重整。
参与对住院患者的多学科团队查房和药师查房,及时解决与用药相关的问题。
(三)临床药学工作
深入临床科室,参与查房、会诊及疑难病例讨论,为临床合理用药提供专业建议。
开展处方点评和医嘱点评工作,促进临床合理用药,持续改进用药质量。
负责药品不良反应(ADR)的监测、收集、上报及分析工作。
为慢性病多重用药患者提供出院后持续随访,确保用药依从性并处理不良反应等问题。
为医务人员提供药品信息服务,开展合理用药培训和宣教。
(四)药品管理工作
协助评估药品订购体系,制定并执行适合本院的药品订购程序,确保药品合理储备。
负责药品的验收、养护、库存管理及有效期监控,确保药品质量与安全。
参与定期药房盘点,确保库存数据准确。
负责麻醉药品、精神药品等特殊管理药品的日常管理,确保符合法律法规要求。
管理药房计算机系统数据更新,维护药品信息的准确性。
(五)静配中心工作
在药师指导下,根据 PIVA 相关规定配制静脉输液,包括全肠外营养液、抗肿瘤药物等。
负责注射药品贴标签、核对及配送工作。
维护静配中心的洁净环境,确保操作符合无菌规范。
任职资格与专业水平要求
(一)基本条件
学历要求:本科及以上学历,取得相应学位。
专业要求:药学、临床药学、医院药学等药学相关专业,专业背景符合医院药学工作需要。
职业资格:持有国家认可的药学专业技术职称(初级西药师及以上),兼具中药师职称和/或主管药师职称者优先;持有执业药师资格证书者优先。
毕业年份与资格:2025—2027 年毕业,本科及以上学历;持有医院药学相关职业资格证书者优先。
(二)专业知识与技能
系统掌握药学基础理论和专业知识,熟悉各类药物的药理作用、临床应用、用法用量、不良反应及药物相互作用。
熟悉药品保管、发放的相关规定和流程,了解医院药事管理相关法律法规。
具备处方审核能力,能够识别和干预不合理处方,保障患者用药安全。
具备良好的临床药学思维,能够参与临床药物治疗方案的评估与优化。
熟悉医院信息系统(HIS)及药房管理系统的操作,能够熟练使用计算机办公软件。
具备一定的药品不良反应监测和报告能力。
(三)综合素质要求
沟通能力:具备良好的中英文沟通能力,能够与患者、医务人员及多学科团队进行有效沟通;能够为外籍患者提供用药咨询服务者优先。
服务意识:以患者为中心,具备良好的服务态度和职业素养,耐心解答患者用药疑问。
责任心:工作严谨细致,具备高度的责任心和安全意识,严格执行操作规程,杜绝差错事故。

The Pharmacy Department of United Family is a professional and international pharmaceutical care team. Pharmacists work beyond the pharmacy itself to become key members of the multidisciplinary medical team, safeguarding patients' medication safety from admission to discharge. The department's signature services include:
Medication Reconciliation: Clinical pharmacists perform medication reconciliation at patient admission and discharge — collecting patients' prescription drugs, over-the-counter medications, nutritional supplements and allergy history, reviewing inpatient orders, and ensuring the accuracy and continuity of medication therapy.
Pain Management: Pharmacists collect patients' pain medication history, conduct opioid tolerance screening, provide input for physicians' pain management plans, and guide nurses in assessing patient pain.
Pre-Prescription Review: Each prescription is reviewed before dispensing to safeguard patients' medication use and intercept potential therapeutic duplications and drug interactions.
Clinical Pharmacy Services: Clinical pharmacists join multidisciplinary team rounds and pharmacist rounds, promptly resolving medication-related issues and providing ongoing monitoring for patients with chronic diseases on multiple medications.
Medication Counseling: Pharmacists provide patients with guidance on prescription drugs, over-the-counter medications and vaccines, explaining in detail how to use medications, precautions and possible adverse reactions.
Scope of Work
(I) Outpatient Pharmacy
- Review and interpret electronic prescriptions from outpatient and emergency departments; assess and resolve actual or potential prescription-related issues.
- Carry out daily dispensing per outpatient pharmacy workflows, including drug preparation, verification, dispensing, returns and management of unpaid orders.
- Provide medication counseling and guidance to patients on prescription drugs, vaccines and over-the-counter medications.
- Participate in daily pharmacy operations, including drug counts, expiry-date checks, inventory management and record maintenance.
(II) Inpatient Pharmacy
- Review medication orders for inpatients to intercept potential therapeutic duplications, drug interactions and inappropriate medication use.
- Prepare, verify and dispense medications for inpatients, ensuring accurate and timely delivery to the wards.
- Collect patients' medication history at admission, including prescription drugs, over-the-counter medications, nutritional supplements and allergy history, and complete medication reconciliation.
- Participate in multidisciplinary team rounds and pharmacist rounds for inpatients, promptly resolving medication-related issues.
(III) Clinical Pharmacy
- Work closely with clinical departments; participate in ward rounds, consultations and difficult-case discussions, providing professional advice on rational medication use.
- Conduct prescription reviews and medical-order reviews to promote rational medication use and continuous quality improvement.
- Monitor, collect, report and analyze adverse drug reactions (ADR).
- Provide ongoing post-discharge follow-up for patients with chronic diseases on multiple medications, ensuring medication adherence and managing adverse reactions.
- Provide drug information services to healthcare professionals; conduct training and education on rational medication use.
(IV) Medication Management
- Assist in evaluating the drug ordering system; develop and implement ordering procedures suited to the hospital to ensure appropriate drug reserves.
- Oversee drug acceptance, storage and maintenance, inventory management and expiry-date monitoring to ensure drug quality and safety.
- Participate in periodic pharmacy stock-taking to ensure accurate inventory data.
- Manage the day-to-day handling of specially controlled medications such as narcotics and psychotropic drugs, ensuring compliance with laws and regulations.
- Maintain data updates in the pharmacy computer system to ensure accurate drug information.
(V) IV Admixture Center (PIVAS)
- Under pharmacist guidance, prepare intravenous infusions in accordance with PIVAS regulations, including total parenteral nutrition (TPN) and antineoplastic drugs.
- Label, verify and deliver injectable medications.
- Maintain the clean environment of the IV admixture center, ensuring operations comply with aseptic standards.
Qualifications & Professional Requirements
(I) Basic Requirements
- Education: Bachelor's degree or above, with the corresponding degree obtained.
- Major: Pharmacy, Clinical Pharmacy, Hospital Pharmacy or other pharmacy-related majors, with a professional background meeting the needs of hospital pharmacy practice.
- Professional Credentials: Hold a nationally recognized pharmaceutical professional title (Junior Pharmacist in Western Pharmacy or above); holders of a TCM Pharmacist title and/or Pharmacist-in-Charge title preferred; holders of the Licensed Pharmacist Qualification Certificate preferred.
- Year of Graduation: Graduates of 2025–2027, Bachelor's degree or above; holders of hospital pharmacy-related professional qualification certificates preferred.
(II) Professional Knowledge & Skills
- Systematic command of fundamental pharmaceutical theory and professional knowledge; familiar with the pharmacological effects, clinical applications, dosage and administration, adverse reactions and drug interactions of various medications.
- Familiar with the rules and workflows for drug storage and dispensing; knowledgeable about laws and regulations on hospital pharmacy administration.
- Prescription review capability: able to identify and intervene in inappropriate prescriptions to safeguard patients' medication safety.
- Sound clinical pharmacy thinking; able to participate in the evaluation and optimization of pharmacotherapy plans.
- Familiar with Hospital Information Systems (HIS) and pharmacy management systems; proficient with common computer office software.
- Capability in adverse drug reaction monitoring and reporting.
(III) Overall Competencies
- Communication: Good communication skills in Chinese and English; able to communicate effectively with patients, healthcare professionals and multidisciplinary teams; able to provide medication counseling to expatriate patients is a plus.
- Service Awareness: Patient-centered, with a good service attitude and professional ethics; patiently addresses patients' medication questions.
- Sense of Responsibility: Rigorous and detail-oriented, with a high sense of responsibility and safety awareness; strictly follows operating procedures to prevent errors and accidents.
